*Job Title: *Home Health & Hospice Nurse
*Location:* Farmington, New Mexico
*Pay:* $60,000–$66,000 per year, depending on experience + $5,000 Sign-On Bonus (terms apply).

*Job Description:*
We seek a dedicated and compassionate Registered Nurse (RN) to join our home health and hospice care team. In this role, you will be at the forefront of delivering high-quality care to our patients, including end-of-life care for our hospice patients. You will work closely with patients, their families, and other healthcare professionals to create and implement comprehensive care plans that meet each patient's needs.
*Responsibilities:*
* Conduct initial assessments of new patients and develop individualized care plans.
* Provide direct patient care by the patient's individualized care plan.
* Provide skilled nursing care, including medication administration, wound care, and monitoring of vital signs.
* Collaborate with a multidisciplinary team to facilitate integrated and comprehensive care.
* Assess, monitor, document, and report the progress of the patient's health and condition.
* Provide emotional support and compassionate care to patients and their families.
* Educate patients and their families on disease management, self-care, and end-of-life planning.
* Coordinate with medical social workers and chaplain to provide bereavement support to families.
* Maintain accurate and timely documentation of patient care.
* Adhere to all company policies, procedures, and regulatory requirements.
*Qualifications:*
* Current and unencumbered RN or LPN license in the state of New Mexico.
* Excellent communication, interpersonal, and organizational skills.
* Ability to work independently and manage time effectively.
* Basic computer skills, including experience with electronic medical records (EMR) systems.
* CPR certification and valid driver's license with reliable transportation.
* Strong problem-solving and decision-making skills.
* Compassionate and empathetic demeanor with a strong dedication to patient care.
