As a Primary Care Physician (PCP) , you will play a crucial role in a collaborative, team-based environment dedicated to providing exceptional medical services to seniors. Your focus will be on delivering coordinated, value-based care that enhances patient outcomes through compassionate interactions and effective communication.

Your Role in Our Team

As a PCP, you will lead our multidisciplinary care team, which includes:

  • Doctors
  • Advanced Practice Professionals
  • Pharm D
  • Care Coach Nurses
  • Medical Assistants
  • Behavioral Health Specialists
  • Quality Based Coders
  • Referral Coordinators

This collaborative approach allows us to provide an unmatched experience for seniors, ensuring higher quality care and better outcomes. With fewer patients to see, you can dedicate more time to each individual, fostering deeper connections and understanding.

Your Key Responsibilities

Patient Care:

  • Evaluate and treat patients in line with established care standards.
  • Monitor patient care quality using available data and chart reviews.
  • Coordinate patient services, including specialty referrals and home health care.
  • Engage actively with the care team during daily huddles.
  • Maintain a professional and approachable demeanor to foster cooperation.
  • Complete medical record documentation promptly, optimizing coding specificity.
  • Participate in discussions about quality of care and outcome data.
  • Contribute to growth opportunities for new or existing services.
  • Participate in the local primary care “on-call” program as needed.
  • Ensure compliance with licensing and accreditation standards.
  • Focus 100% on direct patient care, including administrative tasks related to patient interactions.

What We’re Looking For

Required Qualifications:

  • Graduate of an accredited MD or DO program.
  • Current and unrestricted medical license or willingness to obtain one.
  • Exceptional patient/customer service skills.
  • Basic computer skills, including email and EMR.
  • Excellent verbal and written communication abilities.
  • Strong interpersonal skills with colleagues and patients.
  • Commitment to integrated team-based care.
  • Willingness to adapt to a value-based care setting.
  • Must undergo TB screening if selected for the role.

Preferred Qualifications:

  • Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • At least 2 years of relevant experience.
  • Active and unrestricted DEA license.
  • Experience managing Medicare Advantage patients.
  • Knowledge of Medicare guidelines and HEDIS quality indicators.
  • Bilingual candidates are a plus!
